In-Home Care

Aged Care Services — Compassionate Support That Keeps You at Home

At Sewa Foundations, we understand that ageing and disability can bring new challenges and that the best care is the kind that respects your choices, preserves dignity, and helps you stay living where you feel most comfortable – at home. Our NDIS Aged Care Services combine clinical skill, practical daily support and culturally aware person-centred care so you and the people who love you can live with greater independence and peace of mind.

What can the NDIS fund for people with age-related needs?

The NDIS funds supports that are directly related to a person’s disability and their goals. This includes practical in-home supports such as personal care (showering, dressing), assistance with daily tasks, specialised nursing and community access, plus home modifications that make living safer and more independent. These supports are intended to help participants live ordinary everyday lives.

It’s important to note eligibility rules –

Generally, access to the NDIS is for people under 65 (there are specific transition rules if a participant moves into residential aged care before they turn 65). If someone first enters government-funded aged-care services after their 65th birthday, those aged care services usually become the primary funding source. We can help you understand how these rules apply to individual circumstances

What NDIS aged-care supports can include?

We deliver a rounded package of services all tailored to your NDIS plan and goals –
  • Personal & domestic care: Support with showering, dressing, meal prep, household tasks and mobility assistance.
  • Clinical nursing at home: Wound care, medication management, chronic-condition monitoring and liaison with allied health.
  • Home modifications & equipment: Assessment and coordination for ramps, rails, bathroom adaptations and mobility aids.
  • Community participation: Transport to appointments, social activities and allied-health visits to reduce isolation and promote wellbeing.
  • Transition & placement support: Specialist assistance for younger people in residential aged care who wish to move to more appropriate housing.
Every service is delivered by trained staff, follows NDIS practice standards and where required by registered providers. We prioritise cultural safety, family involvement and consistent staffing so your care feels familiar and safe.

How Sewa Foundations make a difference?

  • Person-first planning: We write plans around your goals; not one-size-fits-all packages
  • Home-first approach: Wherever possible we help you remain in your community with supports that reduce hospital or residential stays
  • Experienced, trusted staff: Skilled support workers and clinically informed care; rigorous screening, training and respectful communication.
  • Individualised care plans: We co-design supports based on your goals, not a one-size-fits-all checklist.
  • Practical home upgrades: We coordinate assessments and approved home modifications and equipment claims under your plan.
  • Smooth transitions from aged care settings: If you or a family member is younger than 65 and in residential aged care, we help explore alternatives and can connect you with the right NDIA teams and housing options.
